Welcome to Geriatric Integrative Services

Our model of service may involve:  

• Intensive Geriatric Management – think of a medical “tune-up”for complex patients with multiple chronic disease diagnosis and polypharmacy, involving interdisciplinary family-aided appointments to reach a specific plan of care and return to primary care for ongoing follow up.
• Geriatrician Consult Intervention: Answer specific concern(s) relating to geriatric care as requested by the referring Physician. Formulate and communicate a clear and specific plan of care for ongoing follow up by the patient’s primary care physician. Usually involves 1-3 visits, and the patient is discharged to primary care physician for ongoing follow-up.
• Geriatric Conference: Where a patient’s intervention is primarily by any of the Specialized Geriatric Services. There is no face-to-face encounter with Geriatric Physician or MD-Patient relationship, but his care was briefly assessed for proper medical recommendations within the team prior remitting a plan of care to Primary care to implement.    
• Extended asunder follow-up: Patients deemed “frail “ or “at risk of decline” by the Geriatrician may qualify for 1-3 aleatory visit(s) in a year to maintain/optimize residual function. The support from Primary care is needed at all times as there is no transfer of care at any moment during our intervention.
